modifierIl me semble que les critères définis par l'ACR et rapportés par Hunder en 1990 étaient plus des critères de "classification" que des critères "diagnostics". C'est à dire qu'ils permettent de classer l'artérite gigantocellulaire par rapport aux autres vascularites, mais ne permettent pas de faire le diagnostics.
Ceci est important car peut conduire à des erreurs d'interprétation: les données de sensibilité et de spécificité indiquées sont données par rapport à une cohorte de patient ayant une vascularite, pas des patients "tout venant" telles qu'ont peut les voire dans un cabinet ou un service hospitalier. Dans une telle population, la sensibilité et la spécificité de ces critères seraient probablement plus faible
(un exemple simple: si je prend 3 critère: age >50 ans, céphalé récente et syndrome inflammatoire, j'ai mes trois critères suffisant pour classifier ma maladie par rapport à d'autre vascularite si je suis sure qu'il s'agit d'une vascularite (preuve à l'histologie ou à l'imagerie. En revanche, si je ne suis pas sure du dianostic de la vascularite, j'ai beaucoup de diagnostics différentiels à éliminer: méningite infectieuse ou carcinomateuse, sinusite, néoplasie cérébrale etc... mes 3 critéres ACR ne sont pas suffisants.)
Il faudrait donc changer les termes "critères diagnostiques" dans cet article, qui sont inappropriés. Qu'en pensez vous?

modifierATTENTION : de nos jours, ce n'est plus la VS qu'on teste prioritairement mais la CRP, indicateur plus spécifique et plus fiable.
Plus accessoirement, dans le § Symptômes, vous pouvez ajouter « paupière tombante ».
